Check your email! After your program/session has ended you will receive an email in 3 to 5 business days. Please take a few moments to provide feedback. This feedback will help guide us for the future.
Note: The survey is limited to one response per computer. If you require immediate assistance on an issue, please contact (630) 232-4542.
